Welcome! Whether you’ve had plenty of massage therapy before, or this is your first time ever, this page is written specifically for you!  If you’ve never had a massage before, it will give you some important information you’ll need to know before your visit to our studio.  If you’ve had a lot of massage before, please understand that every massage therapist is different, so your visit with us may or may not be like any other.

First, please review the Indications and Contraindications for massage and let us know if any of the listed conditions apply to you.  It is important that we’re aware of this information, because there are conditions for which massage may help or hinder, and other conditions that may require us to use additional caution or alter our techniques to provide you with a safe and therapeutic session.

New Client Intake Paperwork

We now provide all the initial office paperwork on the web for your convenience. There are two forms, the New Client Intake Form and our Policy Statement, both of which are provided below.  Please set aside about 10-15 minutes to download, print, and legibly complete them beforehand, and please be sure to bring them with you to your first appointment.

New Client Intake Form (.pdf)

This form is in Acrobat (.pdf) format. After opening the file, you should see three (3) pages. Download and print all three pages. Please read over all information; completely fill in ALL blanks, and sign the last (third) page.

Policy Statement

This form is in Microsoft Word (.doc) format.  Please print it out, read carefully, ask any questions you have, and sign.

Some Helpful Information to know before your first session...

To your first appointment, please bring with you:

  • Your completed intake forms and signed policy sheet.
  • Cash so that you may take advantage of the 5% cash discount (reflected in the prices on our Services page).
  • Your calendar and work schedule so that you can schedule further appointments in advance with confidence.
  • The original printed gift certificate, if you are receiving your massage as a gift.
  • Any MRIs, x-rays, CT scans, bone scans, or other diagnostic imaging if you happen to have any in your possession--these may be very helpful!
  • Optional: a set of casual clothes to change into after your massage, especially if you’re coming straight from work.
  • Optional: your contact lens case so that you may remove your contact lenses during your massage.

Basic First-Timer Information

Before your Session:
It’s extremely important that you drink plenty of plain, pure water throughout the day before your massage.  If you don’t get enough water, you won’t benefit nearly as much from your massage.  We address the common water questions specifically here; just scroll down a little.

Please abstain from all intoxicants for at least 24 hours prior to your session to avoid unwanted and uncomfortable after-effects.  (Alcohol consumption is a strict contra-indication to massage therapy!)

Please let us know if you have a preference for--or can’t stand--any particular type of music, linen material, scent, or technique. Please tell us if you like a lot of oil used, or if you would like us to use as little as possible.  Please tell us if you have allergies to any oils, scents, dust, or cats. Please inform us of any contraindications you have, or if you have any conditions we need to know about in order to provide you with a better visit.  If you’ve had a massage before, feel free to tell us about what you liked or didn’t like about past experiences and we’ll be happy to try to duplicate a positive experience, or avoid repeating a negative one.

Arrival:
Please plan to arrive 15 minutes early to your first appointment.  If you are early, please kindly wait in your vehicle until the white front door opens from the inside; this protects both your privacy and that of those before you.  Once the door is open, feel free to come on up and ring the bell. If you’re early and you’d like to see if we can start your session early, please call ahead first and talk to one of us in person before coming up to the door.  We’ll be happy to start early if we can. If you’re going to be late, please let us know as soon as you can, out of courtesy!  If you do so, we can try to arrange to give you all the time we can spare. If you’re really late, we can use the remaining time to work on the most pressing issues. The regular session fee applies.

Complimentary Consultation:
Before we get started, we’ll take a few minutes to go over the information on your paperwork, discuss what you hope to gain from therapy (both today and over the long-term), and talk. As long as you’re 15 minutes early to your session, any consultation time we take past 15 minutes does not cut into your actual session time.
